Patient Information Appointments

Our office will contact you at least 24 hours in advance to remind you of your scheduled appointment. Please bring a list of your current medications or the medications themselves and the names of your specialist physicians with you to the appointment. If you cannot make your appointment, we encourage you to cancel at least 24 hours in advance. If you fail to cancel or show for an appointment, you will be charged $25.00. We ask for the consideration to cancel appointments to open the appointment up for someone needing attention due to an acute illness.

The Patient's Responsibilities

You need to be honest with us regarding your health, family history, and lifestyle to provide the highest level of medical care. Many diseases are adversely affected by lifestyle choices. Our job is to educate you about how your lifestyle affects the disease process, but it is your responsibility to implement lifestyle changes. We will give you support and encouragement, but it is up to you to follow through. You are responsible for scheduling follow-up appointments for tests or specialist physicians.

Billing

Our office will file your insurance claim with your primary insurance company and, if applicable, secondary insurances. It is your responsibility to provide us with current insurance information so claims will be paid on time. The patient is responsible for claims not reimbursed by your insurance company, and you will receive a monthly statement indicating your balance. Co-pays, deductibles, and coinsurance will be due at the scheduled appointment for specialty procedures in our office, and we will notify you of associated coinsurances and deductibles. If you have questions regarding your account, please contact our billing department. If you seek treatment for an injury related to worker's compensation or personal injury, notify the billing office. The billing office will need information regarding the injury to file the claim with the proper entity.

Medication Refills

When you need to refill a prescription, call the pharmacy where the prescription was last filled at least 24 hours before needing the medication. When necessary, the pharmacy will contact our office regarding the refill. We respond to pharmacy requests received prior to 3:30 pm by the end of the business day. Requests received after 3:30 pm will be addressed the following business day. If you need a prescription refill at the time of your office visit, please inform the nurse while you are in the treatment room.
